Survey Instructions
Public Schools, Diocesan Schools, Charter Schools, and
State Special Schools

ALL K-12 schools must complete this survey no later than May 1, 2004.
Subsequent funding for technology will depend on the completion of these forms.

A school-based person (i.e. principal or school technology coordinator), not
the district technology coordinator, should complete this survey.

The survey must reflect actions and purchases for the 2003-2004 school year and
should be based on the status of your school on the day of the submission of
the survey.

Before beginning the online survey, you should gather all necessary data using
a hard copy version of the survey available at http://www.teachlouisiana.net/survey/survey_intro2004.htm
The online submission will be simpler and quicker if you have already done your
data collection using the paper version of the survey.

To access the survey:

Select the name of your system (public school district, diocesan system,
special state school, or charter school) from the dropdown list.

Select the name of your school from the dropdown list.
-
Enter your password. Your school password was emailed to the district
technology coordinator for your system and/or to the principal of your school.
If you did not receive your password, please contact the following people:

Public and diocesan schools: contact the technology coordinator for your school
system.

Charter schools and state special schools contact Cyn Bertrand (cyn.bertrand@la.gov).

After entering your password, click the Submit button.
-
Passwords are available through your district's technology coordinator).

After completing each section, click the "submit"
button to submit that data and continue to the next section. You do not need to
submit all sections in one sitting.

Once you have submitted a section, the "submit" button will no
longer be available for the submitted section. You will only have the ability
to view the data you have entered and continue to the next section in the
survey by clicking the "next page"
button.

If you find an error in your data after submitting a section, please contact
your district's technology coordinator to fix the error.

If an error message appears when you click the submit button, one or more
fields were not completed or not completed correctly. The red triangle with (!)
will denote these fields. Simply supply the missing information or corrections
then click submit
again.

Do not use commas, decimal points or dollar signs when entering numbers.

Independent, Non-Public Schools

as your system name.

Select the name of your school from the dropdown list.

Enter your password. Your school password was emailed to the principal of your
school. If you did not receive your password, please contact Cyn Bertrand (cyn.bertrand@la.gov).

After entering your password, click the Submit button.

Passwords have been emailed to the school's principal. However, if you need
assistance in obtaining an access code, please contact Cyn Bertrand (cyn.bertrand@la.gov).
