Louisiana System Technology Survey Instructions

  1. All parish/city school systems, diocesan systems, and special state schools must complete this survey no later than July 1, 2005. Subsequent funding for technology will depend on the completion of this instrument.
  2. Before a system can begin filling out its survey, all school surveys must be submitted and approved by the system.
  3. The system survey should be completed by the School System Technology Coordinator and must reflect actions and purchases for the 2004-2005 school year.
  4. Complete the survey based on the status of your school system on the day you submit the form.
  5. Do not use commas, decimal points or dollar signs when entering numbers.
  6. If an error message appears, one or more fields were not completed or not completed correctly. The red triangle with (!) will denote these fields. Supply the missing information or corrections then click Submit.
  7. The school system survey has been designed to be submitted by sections. When you begin a section, you will need to complete the entire section and click the submit button to submit your data. PLEASE NOTE: Once you have submitted your data, you will not be able to go back and edit data.
  8. After submitting each survey section, your completed survey section will appear and can be printed.
