INSTRUCTIONS FOR LOGGING INTO THE SURVEYS:
Schools:
Every K-12 school in Louisiana must complete a school-level technology survey. Additionally, any nonpublic school, state special school, or charter school that has received or will receive a 2008-2009 CBTF grant must fill out the CBTF grant evaluation report. School submitters should follow the log-in procedure below:

Select the name of your system (public school district, diocesan system, special state school, or charter school) from the dropdown list. Independent nonpublic schools will select the name “independent nonpublic schools” as your system name.

Select the name of your school from the dropdown list.

If your system or school was not included in the dropdown lists, or the name has changed, or a school has closed, please contact Leslie Copelin (see next item.)

Enter your password. If you did not receive your password, please contact the following people: Public and Diocesan Schools -- contact the technology coordinator for your school system. Charter schools, state special schools, and independent nonpublic schools, contact Leslie Copelin at Leslie.Copelin@ladoe.org

After entering your password, click the Sign In button.
Public School Systems and Diocesan Systems:
Every public school system and diocesan system must complete a system-level technology survey and evaluation report (state specials schools, charter schools, and recovery school districts are exempt from this requirement.) Additionally, any system that has received or will receive an EETT, CBTF, or HighTech grant must complete the appropriate grant evaluation report(s). System submitters should follow the log-in procedure below:

Select your system's name from the dropdown list.

Enter the password for your system.

After entering your password, click the Submit button.
