INSTRUCTIONS FOR LOGGING INTO SELF ASSESSMENT SURVEYS:

Students, Teachers, Administrators, and Central Office Administrators Assessments:

Every K-12 school in Louisiana has been issued a password for their school's students, teachers and administrators:

Select the assessment.
Select the name of your system (public school district, diocesan system, special state school, or charter school) from the dropdown list. If you are an independent, nonpublic school (not part of a public district or diocesan system, not a charter school, and not a state special school), then select the name “independent nonpublic school” as your system name.
Select the name of your school from the dropdown list.
Enter the appropriate password.
After entering your password, click the Submit button.
Hardcopies of the surveys are available! Please log into our Report Request System.
If an error occurred during an assessment survey, the person should log in with a new ID and slightly different name and start over. Most issues are due to the application "timing out" and simply closing out and restarting tends to fix the issue. When all surveys are completed a listing report request should be made to check for potentially invalid surveys counting against the school or district. The report we email will give instructions on how to remove erroneous results.
