INSTRUCTIONS FOR LOGGING INTO SELF ASSESSMENT SURVEYS:

Due to requests to expedite the production of the new tool, we have disabled this site. We apologize for any inconvenience. Thank you for your participation!! We are processing the results to help us design a new tool for the 2008-2009 school year and beyond.

Entities participating in this pilot have been issued special passwords for their students.

Select the name of your system (public school district, diocesan system, special state school, or charter school) from the dropdown list. Independent nonpublic schools will select the name “independent nonpublic schools” as your system name.
Select the name of your school from the dropdown list.
Enter your password. If you did not receive your password, please contact Joshua Riddell at Joshua.Riddell@ladoe.org
After entering your password, click the Sign In button.
Students are required to complete the survey in one seating. If this is not possible, the student will need to take the survey at a different time. Only completed surveys are counted, while incomplete surveys are erased.

NOTE: This is a pilot version with minimal features. These surveys will be used to refine and develop a full version. Though the results will be displayed immediately after the student completes the assessment, please remember that this pilot is NOT yet a fully validated instrument. Results should NOT be used for reporting proficiencies on the 2007-2008 School Technology Surveys. Accordingly, no listing reports or summary reports are available with this pilot.
